In-Home Residential Habilitation Services (Res-Hab)

"In-Home Residential Habilitation" is a service designed to assist an individual in maximizing their potential for independence, inclusion and productivity within their home and community. A Residential Habilitation Specialist will work with the individual on valued outcomes that have been determined by the person being served and/or their advocate. Activities will include the development of adaptive living skills, community inclusion activities and other skills that will increase independence, choice and inclusion within one's home and community. In-Home Residential Habilitation is appropriate for individuals of all ages with a developmental disability.

Medicaid Service Coordination (MSC)

Service Coordination is the key to gaining access to services and supports for people with developmental disabilities. The Service Coordinator serves as the individual's agent in promoting a person centered agenda that centers around Independence, Individuality, Inclusion, and Productivity. The service coordinator will initiate the intake, obtain the necessary assessments, make referrals, and link and monitor the individual's ongoing services.

Non-Medicaid Service Coordination

Non-Medicaid Service Coordination provides opportunities for individuals with developmental disabilities who do not have Medicaid to receive assistance with obtaining services and supports. The Service Coordination will assist the individual in securing entitlement and will provide linkage and referral to OMRDD Family Support Services and other support services.

After School Recreation

After School recreation provides scheduled respite to both the family members as well as individuals being served. School aged children will enjoy arts & crafts, music therapy, outings to museums, bowling, and other activities of their choice in a structured habilitative environment.

In Home Behavior Management

In-Home Behavioral Management provides supports to individuals and families, who are residing at home and who present challenging behaviors. Each consumer receives an in home clinical assessment of the challenging behavior(s). Individualized strategies are developed to address the challenging behaviors. Each family receives the support of a trained direct support professional who will work with the consumer and family in implementing the newly developed behavioral management strategies.

Day Habilitation Services

Day Habilitation Services provides individuals with developmental disabilities an opportunity to engage in meaningful habilitative activities that are intended to enable, strengthen and encourage individuals to reach their maximum potential. Special attention is given to aiding each consumer in developing the skills and abilities necessary to achieve independence in the community. Habilitative activities include the development of skills in areas such as, but not limited to: Independent travel, time management, money management, socialization, behavior management, job readiness skills, household maintenance, shopping for food, clothing and household supplies, community integration, educational and academic skills development, developing choices for socialization, leisure and recreation, community awareness and self preservation, personal hygiene and self care skills developing.
